Home Secretary Shabana Mahmood announced an independent review on Monday into the circumstances surrounding the murder of 18-year-old Henry Nowak and the response of public authorities.

Former justice secretary Alex Chalk will lead the review, with an initial report due by March 2027. Nowak was killed in Southampton in December 2025.

The first phase will examine what relevant organisations knew about his murderer during the preceding five years. Its remit includes police, health services, local authorities, educational institutions and faith organisations, and whether warning signs presented opportunities for intervention.

It will also examine how criminal justice agencies treated the Nowak family after the killing. The family welcomed the review as a step towards answers and accountability.

A second phase will follow completion of the Independent Office for Police Conduct investigation into officers who attended the scene. The announcement establishes a process for examining possible failures; it does not itself determine the outcome of that investigation or the review.
